STEP 1:

PAY FOR TEST ON LIFELABS AND PRINT REQUISITION FORM. IF YOU HAVE ALREADY PAID LIFELABS & HAVE PRINTED REQUISITION FORM START AT STEP 2

You must PRINT your LIFELABS REQUISITION FORM and bring it to pharmacy.
You must also bring your
PASSPORT to your appointment to receive the test.

BOOK APPOINTMENT

If there are no appointment slots available we accept walk-ins as well

Please note that your appointment time is your check-in time. Time is still needed for paperwork that the team needs to do before you get your test

PCR TRAVEL COVID-19 testing is available for asymptomatic individuals who require a negative PCR COVID-19 test in preparation for an upcoming international flight. Results are typically available within 48 hours. For same day appointments, if there is no availability on the booking site, please call the store at 416-538-0105 ext.33. If you cannot get through e-mail us at psdm1377@shoppersdrugmart.ca with subject line "TRAVEL COVID TESTING" and include a contact phone number.

Payment and intake form must be filled out from LifeLabs (Link for intake form in step 1 above), Shoppers Drug Mart Does not collect payment. YOU MUST PRINT YOUR LIFELABS REQUISITION FORM AND BRING IT TO THE PHARMACY. YOU MUST ALSO FILL OUT THE ASSESSMENT FORM (LINK IN STEP 2 ABOVE) AND BRING THAT WITH YOU AS WELL.

Shoppers Drug Mart is not responsible for determining the type of test required and the timing required for your test. Please ensure you know how much lead time you require for your test before booking an appointment.

INSTRUCTIONS (DETAILED):

  1. Fill out an intake form and pay for your test through the LifeLabs online portal. Upon payment, you will receive a confirmation email, along with a requisition form. YOU MUST PRINT YOUR LIFELABS REQUISITION FORM AND BRING IT TO THE PHARMACY.


  1. Fill out the Assessment Form: Fill out the assessment form and fill it out. You must answer NO to all questions in order to be eligible for a test. YOU MUST FILL OUT THE ASSESSMENT FORM.


  1. Book Your Appointment using the link below. Do not book until you have paid for your test and have a LifeLab requisition form. The booking tool will only allow you to book at our location at 958 Bloor St. W, Toronto, On, M6H 1L6. For same day appointments, if there is no availability on the booking site, please call the store at 416-538-0105 ext.33.


  1. Come in for your test. Ensure you have your lifelab requisition form printed and assessment form is completed. The requisition form is your proof of payment and a test will not be performed without this form. You must also bring your passport to your appointment to receive the test. You must meet eligibility criteria (Below) in order to receive the COVID-19 test. If you are eligible, please arrive 5 minutes before your appointment.

ELIGIBILITY:

  • Be asymptomatic (you have no symptoms of COVID-19)

  • Have not been in contact with someone known to have COVID-19 within past 14 days

  • Have not travelled outside of Canada within the last 14 days

  • Have not been advised to get tested for COVID-19 through an exposure notification through the COVID-19 app

  • Have not tested positive for COVID-19 in the past 90 days

  1. Get your results! Your results will be provided to you by Lifelabs. We will not have access to test results at the store. You will receive an email with a secure link to access your test results within 48 hours or as soon as feasible.
